All nominations must be submitted using the MoHIMA Awards Nomination Form, completed in its entirety. Nominations must follow the nomination guidelines and must include the supporting materials listed below.

Please note that you will have the ability to save the nomination and return to work on it again at a later time.  You will see "Save and continue nomination later" beginning on the second page.

The nominee's resume and two reference letters must be uploaded on the online nomination form.

All nominations must be received by March 1st to be considered for awards. Awards will be presented at the MoHIMA Annual Meeting.
